REQUIREMENTS FOR THE REPATRIATION OF BODILY/
CREMATED REMAINS OF A DECEASED FILIPINO/FOREIGN NATIONAL

Please prepare the original and one (1) photocopy each of the following requirements for certification and documentation at the Consulate:

  1. Certified true copy of the Report of Death/Death Certificate (Shibo Todoke/Shibo Shomeisho no Kisai Jiko Shomeisho) from the city hall/ward office.
  2. For the repatriation of bodily remains:
    • Certificate of Embalmment (Bofu Shori Shomeisho) executed by the funeral parlor. The certificate must be notarized by a notary public at a Koshonin Yakuba.
    • Letter of Guarantee (Nokan Shomeisho) that the casket contains only the bodily remains of the deceased, executed by the funeral parlor. The letter must be notarized by a notary public at a Koshonin Yakuba.
    • Certificate of Non-Contagious Remains (Hikansen Shomeisho) certifying that the remains of the deceased is not infectious or has no contagious disease. The certificate must be notarized by a notary public at a Koshonin Yakuba.
  3. For the repatriation of cremated remains:
    • Certificate of Cremation (Kaso Shomeisho) or Cremation Permit (Kaso Kyokasho) as long as the place, date and time of execution of cremation are indicated. Please supply an English translation. The Japanese version must be certified by the city hall /ward office and the cremation facility.
  4. Seafarer's Identification and Record Book (Seaman's Book), Statement of Fact, Crew List and other relevant documents if the deceased is a seafarer.
  5. Original passport of the deceased. If the passport is not available, an NSO-certified Certificate of Live Birth of the deceased may be submitted. Said certificate must be authenticated at the Department of Foreign Affairs in Manila (Authentication Area). An authenticated Certificate of Marriage is also needed if the deceased is a married woman.

For all Japanese documents, please provide an English translation with name and signature of the translator and the date of translation of the document. For repatriation of cremated remains, please bring the urn containing the cremated remains. The Consulate will seal the um after inspection of its contents.

SCHEDULE

Applications may be submitted to the Consulate from Monday to Friday, from 9:00 a.m. to 5:00 p.m. (with lunch break from 12:00 p.m. to 1:00 p.m.). Please note that the Consulate is closed during
Philippine and Japanese holidays.
