Kung nagbabalak ang mga biological parents na ikasal sa isa’t-isa, aming inaabiso na magpakasal na muna ang biological parents bago i-report ang birth ng bata nang sa gayon, makikilala na agad na legitimated ang bata at mababawasan ang mga dokumentong kakailanganing i-sumite at bayaran ng mga magulang ng bata. Ngunit, kung nagmamadali at hindi na mahihintay na maikasal muna ang mga magulang ng bata bago i-report ang birth nito, i-click ang link sa ibaba. (boxed)

Ang sumusunod na listahan ay ang mga dokumentong kailangang isumite at bayaran ng mga magulang ng bata matapos magpakasal ng mga magulang:

  • Fill-up ONLINE ROB form and click Submit.  The forms will be printed and signed at the Consulate during the appointment. [Click here for the Online ROB form)
  • Shussei Todoke Kisai Jiko Shomeisho (one original and 4 photocopies/xerox)*
  • PSA/NSO Birth Certificate of Filipino parent/s (one original and 4 photocopies/xerox)
  • PSA/NSO Marriage Certificate or Report of Marriage (ROM) of parents or Konin Todoke Kisai Jiko Shomeisho or Konin Todoke Juri Shomeisho (one original and 4 photocopies/xerox)
  • Four (4) photocopies each of the data page of valid passport of parents (or valid ID for non-Filipino parent e.g. driver’s license). For Filipino parents, make sure that your passport bears your signature on page three (3).
  • Affidavit of Legitimation (4 copies, all with original signature) <<link to Affidavit of Legitimation>>
  • One (1) self-addressed Letterpack 520 or return envelope with 960-yen worth of postage stamps/kitte
  • Affidavit of Delayed Registration if filing is beyond one year from the birth of the child (4 copies, all with original signature) [Download Affidavit of Delayed Registration form here]
  • If the child is already five (5) years old and above, please submit a PSA / NSO Certificate of No Birth Record (one original and 4 photocopies/xerox)
  • If the child being registered has a Japanese or foreign passport, please submit four (4) copies of the datapage of the Japanese/foreign passport
  • Prepare fees to be paid below.

Fees or Amount to be Paid: (Prices are subject to change without prior notice.)

o Report of Birth Fee (with Translation of Shussei Todoke) ¥ 7,000
o Affidavit of Legitimation ¥ 3,500

*PAALALA:
Depende sa pagkaka-assess sa mga dokumentong nai-sumite, maaari pa ring hingan kayo ng karagdagang mga dokumento ng Konsulado bago kayo makapag ROB ng inyong anak.
Kung hindi na makakuha mula sa Shiyakusho o Homu-kyoku ng kopya ng Shussei Todoke Kisai Jiko Shomeisho, tinatanggap din ng Konsulado ang original na Boshi Kenko Techo o Maternal and Child Health Handbook ng bata (original and 4 photocopies/xerox) kung kumpleto ang impormasyon hinggil sa bata at magulang sa nasabing dokumento.
