REQUIREMENTS TO FILE A REPORT OF DEATH (ROD) FOR A FILIPINO NATIONAL WHO DIED ABROAD

FOR FILIPINOS WHO DIED IN WESTERN JAPAN

FOR REPORT OF DEATH

  1. Duly accomplished Report of Death form.
  2. Original copy and three (3) photocopies of the shibo todoke no kisai jiko shomeisho issued by the city hall where the death was registered.
  3. Proof of Filipino citizenship of the deceased:
    1. Original Philippine passport and three (3) photocopies of the passport data page.
    2. If the passport is not available, original copy and three (3) photocopies of the PSA or NSO birth certificate.
  4. Passport or valid identification document of the informant.
  5. Affidavit of delayed registration if the death was not registered/reported to the Consulate within 1 year from the occurrence.

FOR REPATRIATION OF REMAINS OF A FILIPINO NATIONAL

  1. Report of Death (ROD) issued by the Philippine Consulate / Embassy
  2. From the funeral parlor,  the following documents must be duly authenticated by the Japanese Foreign Ministry (Gaimusho)
    1.  Notarized affidavit of embalming (bofu shori shomeisho), or Notarized affidavit of packaging of remains in dry ice for international transportation;
    2. Notarized affidavit of guarantee (nokan shomeisho) that the casket contains only the cremated remains of the deceased; and
    3. Notarized affidavit of non-contagious remains (hikansen shomeisho) certifying that the remains of the deceased is not infectious or has no contagious disease.

FOR SHIPMENT OF CREMATED REMAINS 

  1. FOR FILIPINO NATIONALS 
    1. Report of Death (ROD) issued by the Philippine Consulate / Embassy
    2. Original cremation permit (Kaso kyokasho) issued by the city hall or ward office; and
    3. Passport or valid identification document of the person making the request for the shipment.
  2. FOR JAPANESE AND OTHER FOREIGN NATIONALS 
    1. Copy of the shibo todoke no kisai jiko shomeisho, with English translation and duly authenticated by the Japanese Ministry of Foreign Affairs (Gaimusho) and the Philippine Consulate;
    2. Original cremation permit (Kaso kyokasho) issued by the city hall or ward office, with English translation and duly authenticated by the Japanese Ministry of Foreign Affairs (Gaimusho) and the Philippine Consulate; and
    3. Passport copy of the deceased and the person making the request for the shipment.

Report of Death - 3,250 yen 
Affidavit of Delayed Registration - 3,250 yen
Translation - 3,250 yen
Certification - 3,250 yen
