REQUIREMENTS TO FILE A REPORT OF MARRIAGE (ROM) FOR A FILIPINO CITIZEN MARRIED ABROAD 

IMPORTANT: 

  1. ROM applicants will have to personally appear at the Philippine Consulate General in Osaka to file their ROM applications unless they have previously applied for their LCCM at the same Philippine Consulate General;
  2. Please note that effective 1 January 2021, marriages of Filipino citizens which take place in the following prefectures: 1. Aichi  2. Gifu  3. Fukui  4. Ishikawa  5. Toyama must be filed/registered at the Philippine Consulate General in Nagoya. For more details and to obtain an appointment at the Philippine consulate General in Nagoya, please visit their website at: https://nagoyapcg.dfa.gov.ph/

ROM requirements for Filipinos who personally appeared and applied for their LCCM at the Philippine Consulate General in Osaka:    

  • Check the details on the  four (4) pre-printed ROM forms that were sent by the Consulate after applying for the LCCM.   
  • On the separately provided paper,  write the additional information needed on the details of the marriage including the date and place of marriage, religions of the two parties and the names (in Romaji) of the two witnesses.  
  • Sign the ROM forms 
  • Send the ROM forms and the sheet of paper with the marriage details back to the Consulate together with the following documents:
    • The original and four copies of the Kon-in Todoke Kisai Jiko Shomeisho 
    • Four (4) copies of the datapage of the valid passport of the Filipino national 
    • Four (4) copies of the datapage of the valid passport (driver's license if without a passport) of the Japanese or foreign spouse
    • If the spouse of the Filipino national is Japanese ,   send one (1) original and four copies of the spouse’s Kosekitohon (family register) with detail of the marriage
    • If the spouse is a foreigner but not Japanese,  send one (1) original and four opies of the spouse’s Kon-in Todoke Juri Shomeisho 
    • Self-addressed Letterpack 520 return envelope with the correct address where the applicant’s approved ROM will be mailed 
    • If the Report of Marriage is being filed more than one (1) year after the actual date of marriage,   applicant must also submit an Affidavit of Delayed Registration of Marriage
  • Fees:   
    • Report of Marriage Fee of ¥3250
    • Translation Fee for Kon-in Todoke Jiko Shomeisho of ¥3250
    • Translation Fee for Koesekitohon of ¥3250 (only if spouse is Japanese )
    • Affidavit Fee of 3,250 per document (if submitting an Affidavit of Delayed Registration of Marriage)
  • Send the required documents, return envelope and fees to the Philippine Consulate General in Osaka .  Please note that cash payments must be sent through Genkin Kakitome Service or cash registered mail of the Post Office

ROM requirements for Filipinos who  DID NOT APPLY for their LCCM at the Philippine Consulate General:

  • A confirmed appointment schedule and personal appearance at the Consulate are REQUIRED. 
  • Original and four (4) copies of the Kon-in Todoke Kisai Jiko Shomeisho 
  • Four (4) copies of the datapage of the valid passport of the Filipino national
  • Four (4) copies of the datapage of the valid passport (driver's license if without a passport) of the Japanese or foreign spouse
  • If the spouse of the Filipino national is Japanese ,   send one (1) original and four copies of the spouse’s Kosekitohon (family register) with detail of the marriage
  • If the spouse is a foreigner but not Japanese,  send one (1) original and four opies of the spouse’s Kon-in Todoke Juri Shomeisho 
  • Self-addressed Letterpack 520 return envelope with the correct address where the applicant’s approved ROM will be mailed
  • If the Report of Marriage is being filed more than one (1) year after the actual date of marriage,   applicant must also submit an Affidavit of Delayed Registration of Marriage
  • One (1) copy of the PSA Birth Certificate 
  • For Filipino nationals who were previously married but have been divorced please submit :
    • One (1) original of the Philippine Judicial Recognition of Divorce with Certificate of Finality apostillized by the DFA 
    • One (1) original of the PSA / NSO Marriage Certificate or Report of Marriage with Annotation on the Judicial Recognition of Divorce 
    • One original of the PSA/NSO Advisory on Marriage/s 
  • For Filipino nationals who were previously married but had their marriage annulled, please submit:
    • One (1) original of the Philippine Judicial Recognition of nullity of marriage with Certificate of Finality apostilled by the DFA (red ribbon)
    • One (1) original of the PSA Marriage Certificate / Report of Marriage with the annotation on the nullity of marriage 
    • One (1) original of the PSA/NSO Advisory on Marriage 
  • For Filipino nationals who were previously married but have been widowed, please submit:
    • If death occurred in the Philippines, one (1) original of the PSA Death Certificate of deceased spouse 
    • If death occurred in Japan and deceased spouse is Japanese, One (1) original of the Shibo todoke kisai jiko shomeisho and Kosekitohon
    • One (1) original of the PSA / NSO Marriage Contract or Report of Marriage
    • One (1) original of the PSA/NSO Advisory on Marriage 

*** Please note that all photocopies should be submitted in A4-sized paper

Fees:

  • Report of Marriage Fee of ¥3250
  • Translation Fee for Kon-in Todoke Jiko Shomeisho of ¥3250
  • Translation Fee for Koesekitohon of ¥3250 (only if spouse is Japanese )
  • Affidavit Fee of 3,250 per document (if submitting an Affidavit of Delayed Registration of Marriage)

MAHALAGANG PAALALA:

1. Kakailanganing mag personal appearance sa Philippine Consulate General sa Osaka ang mga sumusunod na ROM applicants:

  1. Ang mga hindi nag-apply ng LCCM application sa Philippine Consulate General sa Osaka.

2. Mula 1 Enero 2021, ang mga kasal na nangyari sa mga sumusunod na prefectures ay kinakailangang i-file o ipa-rehistro sa Philippine Consulate General sa Nagoya:  1. Aichi  2. Gifu  3. Fukui  4.Ishikawa  5. Toyama. Para sa karagdagang mga detalye at pag-schedule ng appointment sa Nagoya, maaaring bisitahin ang website ng Philippine Consulate General sa Nagoya sa https://nagoyapcg.dfa.gov.ph/


ROM requirements para sa mga aplikanteng nag-personal appearance noong sila ay mag-apply ng LCCM sa Philippine Consulate General sa Osaka:

  • I-check na tama ang mga detalye sa apat (4) na pre-printed ROM forms na pinadala ng Konsulado matapos mag apply ng LCCM
  • Ilagay sa nakalakip na separate na papel ang karagdagang impormasyon sa detalye ng kasal tulad ng date and place of marriage, pangalan ng dalawang witnesses sa Romaji at religion (Paalala: Huwag diretsong isulat sa ROM forms ang mga detalyeng ito, gamitin ang nakalakip na separate na papel)
  • Pirmahan ang ROM forms at ipadala sa Konsulado kasama ang mga sumusunod na dokumento: 
    • Ang original at apat (4) na kopya ng Kon-in Todoke Kisai Jiko Shomeisho (Ito ay ang certified true copy ng Kon-in Todoke na ipinasa sa City Hall sa Japan noong nagpakasal ang mag-asawa kung saan nakasulat ang pangalan ng dalawang witnesses at may hanko ng City Hall)
    • Apat (4) na kopya ng datapage ng valid na passport ng Pilipino
    • Apat (4) na kopya ng datapage ng valid na passport (o driver's license kung walang passport) ng asawang Hapon o foreigner 
    • Kung ang asawa ay Japanese,   ipadala ang original at apat (4) na kopya ng Kosekitohon (family register) ng asawa na may detalye ng kasal
    • Kung ang asawa ay foreigner pero hindi Japanese,  ipadala ang  original at apat (4) na kopya ng Kon-in Todoke Juri Shomeisho ng asawa
    • Isang self-addressed Letterpack 520 return envelope na may pangalan at address kung saan ipapadala ang na aprubahang ROM 
    • Kung ang ROM ay ipina-file higit isang taon na makalipas ang kasal,   magpadala din ng Affidavit of Delayed Registration of Marriage
  • Mga Bayarin:   
    • Report of Marriage Fee na ¥3250 at Translation Fee para sa Kon-in Todoke Jiko Shomeisho na ¥3250 (Total:¥6500)
    • Dagdag na ¥ 3250, kung ang asawa ay Japanese,  Translation Fee para sa Kosekitohon na ¥3250 
    • Dagdag na ¥ 3250 para sa Affidavit Fee kung merong Affidavit of Delayed Registration of Marriage
  • Ipadala ang bayad sa pamamagitan ng Genkin Kakitome Service o cash registered mail ng Post Office
  • Ipadala ang lahat ng dokumento kasama ang return envelope at bayad sa Philippine Consulate General in Osaka 

ROM requirements para sa mga aplikanteng hindi nakapag-apply ng LCCM sa Philippine Consulate General sa Osaka:

  • Ang confirmed appointment schedule at personal appearance sa Konsulado ay REQUIRED
  • Original at apat na kopya ng Kon-in Todoke Kisai Jiko Shomeisho
  • Kung ang asawa ay Japanese ,   ipadala ang original at apat (4) na kopya Kosekitohon (family register) ng asawa na may detalye ng kasal 
  • Kung ang asawa ay foreigner pero hindi Japanese,  ipadala ang original at apat na kopya ng Kon-in Todoke Juri Shomeisho ng asawa 
  • Self-addressed Letterpack 520 return envelope na may pangalan at address kung saan ipapadala ang na-aprubahang ROM 
  • Kung ang ROM ay ipina-file higit isang taon na makalipas ang kasal, magpadala din ng Affidavit of Delayed Registration of Marriage
  • Isang (1) kopya ng PSA Birth Certificate apostillized by the DFA
  • Para sa mga Pilipino na dati nang kasal at na-divorce na dito sa Japan, paki submit ang mga sumusunod:
    • Isang (1) original na Philippine Judicial Recognition of Divorce na may Certificate of Finality na na-apostille ng DFA
    • Isang (1) original na PSA / NSO Marriage Certificate o Report of Marriage na may Annotation on the Judicial Recognition of Divorce
    • Isang original na PSA/NSO Advisory on Marriage/s 
  • Para sa mga Pilipino na dati nang kasal at na-annul ang nasabing kasal, paki submit ang mga sumusunod:
    • Isang (1) original na Philippine Judicial Recognition of nullity of marriage na may Certificate of Finality na na-apostille ng DFA
    • Isang (1) original na PSA Marriage Certificate / Report of Marriage na may annotation ng nullity of marriage 
    • Isang (1) original na PSA/NSO Advisory on Marriage
  • Para sa mga Pilipino na dati nang kasal at nabalo o nabiyuda, paki submit ang mga sumunod:
    • Kung ang dating asawa ay namatay sa Pilipinas, isang (1) original na PSA / NSO Death Certificate ng dating asawa
    • Kung ang dating asawa ay namatay sa Japan isang (1) original na Shibo todoke kisai jiko shomeisho at Kosekitohon
    • Isang (1) original na PSA / NSO Marriage Contract or Report of Marriage
    • Isang (1) original na PSA/NSO Advisory on Marriage

      *** Lahat ng photocopies na i-susumite ay dapat nasa A4-size

  • Mga Bayarin:   
      • Report of Marriage Fee na ¥3250 at Translation Fee para sa Kon-in Todoke Jiko Shomeisho na ¥3250 (Total:¥6500)
      • Dagdag na ¥ 3250, kung ang asawa ay Japanese,  Translation Fee para sa Kosekitohon na ¥3250
