IMPORTANT NOTE:
 
All applications for Legal Capacity to Contract Marriage (LCCM) require the personal appearance of the applicant at the Philippine Consulate General.

Requirements for Filipino citizens: 

  • Duly accomplished and signed form applying for an LCCM.  Form may be downloaded from the website here or obtained at the Consulate-General
  • Applicant's original valid Philippine passport and one photocopy of the data page of the passport 
  • Applicant's Birth Certificate issued by the Philippine Statistics Agency (PSA) (one original).   
  • Two (2) passport size photos of the applicant
  • Proof of visa status: One (1) photocopy of residence  card or Japanese visa
  • Self-addressed, Letter Pack 520 return envelope with the correct address where the LCCM will be mailed 
  • Processing fees for LCCM, translation and affidavits 
  • If the applicant is SINGLE (never married):
    • LCCM Affidavit for Single Applicants.  Click here for form.
    • Certificate of Non-appearance in Marriage Registry (CENOMAR) "FOR MARRIAGE" issued by the Philippine Statistics Office (PSA).  Please ensure that all entries in the CENOMAR are correct and consistent with the PSA birth certificate and passport.       
    • If applicant is 18-20 years old,   submit an Affidavit of Parental Consent (with copies of passports of parents) .  If parents are in the Philippines,  the Affidavit of Parental Consent must be duly notarized and apostilled by the DFA before it can be submitted to the Consulate General.
    • If applicant is 21-25 years old,   submit an Affidavit of Parental Advice  (with copies of passports of parents).    If parents are in the Philippines,   the Affidavit of Parental Advice  must be duly notarized and apostilled by the DFA before it can be submitted to the Consulate General.  
    • If the applicant is a WIDOW or WIDOWER:
      • LCCM Affidavit for Widow/Widower Applicants.  Click here for form.
      • Death certificate issued by the Philippine Statistics Authority (PSA)  if the deceased spouse of the applicant was a Filipino or
      • Death certificate from City/Ward office and Koseki Tohon if the  deceased spouse of the applicant was Japanese;
      • Marriage Certificate issued by the Philippine Statistics Authority (PSA);
      • Advisory on Marriage issued by the Philippine Statistics Authority (PSA);

    (Please note that widows/widowers must wait 300 days after death of spouse before applying for the LCCM)

  • If the applicant is DIVORCED or has an ANNULLED MARRIAGE:
    • LCCM Affidavit for Divorced Applicants.  Click here for form  or
    • LCCM Affidavit for Applicants with Annulled  Marriage.  Click  here for form.
    • Philippine Judicial Declaration of Foreign Divorce (for divorced) or Judicial Decree of Nullity of Marriage (for annulled) with Certificate of Finality  issued by a competent Philippine Court and apostilled by the DFA; 
    • Annotated Marriage Certificate issued by the Philippine Statistics Authority (PSA); 
    • Advisory on Marriage issued by the Philippine Statistics Authority (PSA).

Requirements for Japanese nationals: 

  • LCCM Affidavit for Japanese applicants.  Click here for form.
  • Passport or Driver's License or National Identification Card (original and 1 photocopy) 
  • Koseki Tohon indicating previous records of marriage/divorce/death of spouse, if any
  • Juminhyo
  • Two (2) passport size photos
  • Processing fees for LCCM, affidavits, translations, if any        

Requirements for non-Japanese foreign nationals: 

  • Passport (original and 1 photocopy) 
  • Duly accomplished form applying for an LCCM
  • Residence card (original and 1 photocopy)
  • One (1) passport size photo
  • Certificate of Legal Capacity to Contract Marriage (or its equivalent) from his/her Embassy or Consulate in Japan.  Certificate must be in English or accompanied by an official English translation.  For Brazilian nationals,   please submit Civil Status authenticated by the Brazilian Consulate. 
  • Processing fees for LCCM, affidavits, translations if any

MAHALAGANG ANNOUNCEMENT: Ang lahat ng aplikante para sa Legal Capacity to Contract Marriage (LCCM) ay kinakailangang mag persona appearance sa Philippine Consulate General.

Requirements para sa mga aplikanteng Pilipino:MGA REQUIREMENTS SA PAGKUHA NG LEGAL CAPACITY TO CONTRACT MARRIAGE (LCCM) 

  • Application form na maayos na sinagutan at pinirmahan ng aplikante. Ang application form ay maaring i-download dito
  • Valid passport ng aplikante at photocopy ng data page ng nasabing passport 
  • Original na PSA Birth Certificate ng aplikante (1 original);
  • Dalawang (2) passport size photo ng aplikante
  • Patunay ng visa status sa Japan : Isang photocopy ng residence card o Japanese visa
  • Japan Post Letterpack 520 na envelope. Isulat sa envelope ang buong pangalan , mobile number at address kung saan ipapadala ang LCCM 
  • Processing Fees para sa LCCM, translation at affidavits kung meron
  • Kung ang aplikante ay SINGLE (di pa kinakasal): 
    • LCCM Affidavit para sa mga SINGLE na aplikante.  Ang form ay maaaring ma-download dito.
    • Certificate of Non-appearance in Marriage Registry (CENOMAR) "FOR MARRIAGE" na na-issue ng Philippine Statistics Office (PSA). Siguraduhing na ang lahat ng detalye sa CENOMAR ay tama at tugma sa mga detalye sa PSA birth certificate at sa passport. 
    • Kung ang aplikante ay nasa edad na 18-20 años, kailangan mag submit ng Affidavit of Parental Consent (kasama ang kopya ng passport ng mga magulang) . Kung ang mga magulang ay nasa Pilipinas, ang Affidavit of Parental Consent ay kailangan ma notaryohan muna at ma apostille ng DFA bago ito ibigay sa Konsulado.
    • Kung ang aplikante ay nasa edad na 21-25 años, kailangan mag submit ng Affidavit of Parental Advice (kasama ang kopya ng passport ng mga magulang) . Kung ang mga magulang ay nasa Pilipinas, ang Affidavit of Parental Advice ay kailangan ma notaryohan muna at ma apostille ng DFA bago ito ibigay sa Konsulado.

Paalala: Kailangang mag hintay ng 300 na araw mula sa pagkamatay ng asawa bago makapag-apply ng LCCM

  • Kung ang aplikante ay byuda o balo:
    • LCCM Affidavit para sa mga BYUDA o BALO.  Ang form ay maaaring ma-download dito.
    • Kung ang namatay na asawa ay Pilipino, kailangang i-submit ang kanyang PSA death certificate. 
    • Kung ang namatay na asawa ay Japanese, kailangang i-submit ang kanyang death certificate na galing sa City Hall/Ward Office at ang Koseki Tohon
    • PSA Marriage Certificate 
    • Advisory on Marriage na na-issue ng PSA
  • Kung ang aplikante ay kasal dati pero ngayon ay divorced o annulled na:
    • LCCM Affidavit para sa mga DIVORCED.  Ang form ay maaaring ma-download dito. o
    • LCCM Affidavit para sa mga aplikanteng ANNULLED na ang dating kasal.  Ang form ay maaaring ma-download dito .
    • Philippine Judicial Declaration of Foreign Divorce (para sa mga divorced) o Judicial Decree of Nullity of Marriage (para sa mga annulled) na may Certificate of Finality na na-issue ng isang Korte sa Pilipinas at na apostille ng DFA. 
    • PSA Marriage Certificate na na-apostille ng DFA
    • Advisory on Marriage na na-issue ng PSA at na-apostille ng DFA

Requirements para sa mga aplikanteng Japanese:

  • LCCM Affidavit para sa mga aplikanteng Japanese.  Ang form ay maaaring ma-download dito.
  • Passport or Driver's License or National Identification Card (original and 1 photocopy) 
  • Koseki Tohon na nagpapakita ng kasal / divorce / pagkamatay ng asawa (kung meron naging asawa)
  • Juminhyo
  • Dalawang  (2) passport size na photo
  • Processing fees para sa LCCM, affidavits, translations, kung meron          

Requirements para sa mga Foreigner na hindi Japanese 

  • Passport or Driver's License or National Identification Card (original and 1 photocopy) 
  • Application form na maayos na sinagutan at pinirmahan ng aplikante
  • Residence card (original at isang photocopy)
  • Isang (1) passport size na photo
  • Certificate of Legal Capacity to Contract Marriage na galing sa Embassy or Consulate ng foreigner sa Japan.  Ang certificate ay dapat nasa English o may kasamang English translation.   Para sa mga Brazilian,   paki submit ang  Civil Status na na authenticate ng Brazilian Consulate.
