- Proof of Filipino citizenship and identity, which may be any of the following:
- Copy of the old passport;
- Original PSA birth certificate;
- Philippine-government issued ID cards like SSS, voter’s ID, LTO license
- Three (3) pieces colored photo with the following size: 3.5 cm. x 4.5 cm.
- Affidavit or Sinumpaang Salaysay explaining the circumstances why a Travel document is being applied for; and
- Proof of urgency of travel.
- Payment of processing fee JPY3,900, and other applicable fees.
