1. Personal appearance
2. Proof of Filipino citizenship and identity, which may be any of the following:
   1. Copy of the old passport;
   2. Original PSA birth certificate;
   3. Philippine-government issued ID cards like SSS, voter’s ID, LTO license
3. Four (4) pieces of colored photos with the following size: 3.5 cm. x 4.5 cm.
4. Affidavit or Sinumpaang Salaysay explaining the circumstances why a Travel document is being applied for; and
5. Payment of processing fee JPY4,500, and other applicable fees.
