Requirements for First Time Passport Application :

1.       Personal appearance of applicant.

2.       Duly accomplished passport application form

3.       Original and photocopy of the applicant’s Report of Birth or DFA authenticated PSA (NSO) Birth Certificate.

4.       Self addressed return envelope with Yen 930 postage stamps.

5.       Passport Fee of Y 7,800 (cash only)

ADDITIONAL REQUIREMENTS:

For minor applicants (below 18 years old):

1.       Valid passport of the parents or legal guardian and photocopy of the data page.

2.       Original and photocopy of the parents’ DFA Authenticated PSA (NSO) Marriage Certificate or Report of Marriage.

3.       The minor applicant must be accompanied by the Filipino parent or legal guardian.

NOTE: The Consular Officer reserves the right to require additional proof or documents, pursuant to R.A. 8239 (The Philippine Passport Act of 1996) for the purpose of verifying the identity, citizenship and personal circumstances of the applicant.
