Para sa Legal Guardian o Self Reporter, ang unang-unang kakailanganin ninyong ma-establish at mapatunayan ay kung ang isa ba o parehas sa biological parents ng bata ay Filipino citizen/s noong panahong ipinanganak ang bata.
Upang mapatunayan ang pagiging Filipino citizen ng isa o parehas sa mga biological parents ng bata, kakailanganin ang mga sumusunod na dokumento:
- PSA/NSO Birth Certificate of Filipino parent/s ng bata
- Kopya ng data page of valid Philippine passport of Filipino parent/s
Kakailanganin din ma-establish o mapatunayan na ang batang nire-report ang birth ay siyang tunay na anak din ng nasabing biological parents. Upang mapatunayan ito, kakailanganin din ang isa mga sumusunod na dokumento:
- Shussei Todoke Kisai Jiko Shomeisho ng bata
- Koseki tohon ng magulang kung ang isa sa mga biological parents ng bata ay Hapon
- Boshi Kenko Techo o Maternal and Child Health Handbook ng bata
Kung over 5 years old na ang bata, kakailanganin ding mag-submit ng PSA/NSO Certificate of No Birth Record.
At kung may Japanese o foreign passport din ang bata, kakailanganin din ng kopya ng data page nito.
Basic requirements po ang mga dokumentong nakalista sa itaas. Depende sa sitwasyon ng aplikante, maaaring hingan pa kayo ng karagdagan pang mga dokumento.
Kakailanganin talagang ma-meet ang basic requirements sa itaas bago maka-proceed sa next steps. Kapag nakalap na ang mga basic documents sa itaas o kahit kulang o hindi makumpleto ito matapos ang pagsusumikap na makumpleto ang basic documents sa abot ng makakaya, maaaring makipag-sodan o kumonsulta sa Konsulado at humingi ng appointment kung kinakailangan upang mas masuri ang background ng kaso. Magpunta lamang sa website. Kung gamit ang mobile phone, i-press ang MENU tapos ang Consular Services. Kung gamit ang computer, dumerecho sa Consular Services. Piliin ang Appointment Systems at i-click ang Other Consular Services Appointment System. Magpa-appointment sa Civil Registry & LCCM at dalhin ang mga nakalap na dokumento pati ang proof ng inyong pagiging Legal Guardian o Self Reporter (hal. Passport, ID, o court order) sa takdang araw at oras ng appointment.

For Legal Guardians or Self Reporters, the first thing that you will have to establish and prove is if one or both of the biological parents of the child were Filipino citizens at the time of the birth of the child.
To do this, you will need to be able to submit the following documents:
- PSA/NSO Birth Certificate of the Filipino parent/s of the child
- Copy of the data page of the valid Philippine passport/s of the Filipino parent/s
You will also need to establish and prove that the child whose birth you are reporting is the true biological child of the said biological parents. To do this, you will need to also be able to submit one of the following documents:

The above documents are just the basic requirements needed. Depending on the situation of the applicant, it is possible that additional documents might be required.
