A foreign national proceeding to the Philippines to engage in any lawful occupation, whether for wages or salary or for other forms of compensation where bona fide employer-employee relations exist. They may be professors and teachers for educational institutions, doctors and nurses for hospitals, scientists, professionals and other workers for banking, commercial, industrial, agricultural and other business enterprises.
Note: Prior authorization from the Department of Foreign Affairs in Manila is required.

REQUIREMENTS
Employer in the Philippines: 1. Must file an application for a pre-arranged employment at the Bureau of Immigration in the Philippines.
Employee / Visa Applicant in Japan 1. Duly accomplished Non-Immigrant Visa Form 2. Original and one (1) photocopy of passport data page valid for at least six (6) months beyond the intended period of stay in the Philippines. 3. Original and one (1) photocopy of Japanese Residence Card. For Japanese nationals, you may submit a valid driver’s license and/or My Number. 4. Duly accomplished Personal History Form 5. Duly accomplished Visa Undertaking Form 6. Letter of Endorsement from the employer / company based in the Philippines duly notarized by a Philippine notary public. 7. Police clearance issued by the Japanese police authorities with apostille certificate and English translation. 8. Original with apostille certificate Medical Examination Form FA Form 11 duly accomplished by a General Practitioner (if contemplated period of stay in the Philippines exceeds 1 year). This includes the x-ray results in CD, laboratory reports, and other necessary documents. The certificate will be acceptable within six (6) months from the date of the examination was conducted.
